CLARIFICATIONS TO MEDICAL POLICIES:

  • Autografts and Allografts in the Treatment of Focal Articular Cartilage Lesions
  • Coronary Computed Tomography Angiography with Selective Noninvasive Fractional
  • Flow Reserve
  • Continuous or Intermittent Monitoring of Glucose in Interstitial Fluid
  • LUXTURNA™ (voretigene neparvovec), gene therapy for the treatment of vision loss due to confirmed biallelic
  • RPE65-mediated inherited retinal disease (IRD)
  • Hematopoietic Cell Transplantation for Non-Hodgkin Lymphomas
  • Yescarta™ (axicabtagene ciloleucel) CAR T-cell therapy for treatment of adult patients with relapsed or refractory large B-cell lymphoma
